At the top of your M.A.P. goal setting template, write your Ultimate Outcome.

Ask yourself:

  • What exactly do I want to achieve?
  • By when do I want to achieve it?
  • How will I know I’ve succeeded?


Make it specific and measurable.

Example statements:

  • Increase monthly online sales from $5,000 to $7,500 by December 31.
  • Lose 15 pounds and run a 5K without stopping by October 1.
  • Promote to Frontline Supervisor within 9 months.


This single statement becomes the focus of your personal goal action plan template.

Step 2 – Define Your Purpose and Motivation

Next, capture your Purpose and Motivation. This is where you decide whether you’re truly willing to pay the “price of admission” for your goal.

Ask yourself:

  • Why is this outcome important to me, my family, my team or my business?
  • What will it cost me if I do not achieve it?
  • How will my life or work be different when I succeed?


Write 2–4 sentences that matter to you emotionally, not just logically.

For example:

Step 3 – Identify High‑Impact Activities

Backup Plan:Block 90 minutes each morning before email or social media.


Barrier:I’m afraid of rejection on sales calls.

Backup Plan:Use a simple script, aim for conversations not ‘closes,’ and debrief each call to learn.

Step 5 – Build Your 5‑Day Planner for Success
